PreferredOne does not disclose any nonpublic personal information about current or former
customers to anyone, except as permitted by law.
This Customer Notice describes the internet policies of PreferredOne Community Health Plan,
PreferredOne Administrative Services, Inc. and PreferredOne (collectively referred to as “PreferredOne”).
It serves as a standard for all PreferredOne employees in the collection, use, retention and security
of individual customer information.
PreferredOne collects nonpublic personal information about you from the following sources:
- Applications, enrollment forms or other forms, which include information such as your name, address, birthdate and social security number;
- Your transactions with us, our affiliates or others, such as accessing our web site or seeking health care coverage;
- Third parties, such as previous insurance carriers, licensed insurance brokers or your employer.
"Nonpublic personal information" is nonpublic information about you that we may obtain in connection
with providing a service to you. It may include information about your eligibility for health care
benefits, your claims history, your medical history or other information relating to the maintenance
or administration of your health care benefits.
We may provide nonpublic personal information that we collect to affiliated or nonaffiliated
parties involved in underwriting, processing, servicing and marketing our insurance related
products and services. Except as permitted by law, we will not disclose your nonpublic personal
information to nonaffiliated third parties, unless we first obtain your authorization or offer
you an opportunity to refuse or “opt out” of such disclosure.
We restrict access to nonpublic personal information about you to those employees who need to know
that information to provide services to you. We maintain physical, electronic and procedural
safeguards to protect your nonpublic personal information.

Anonymous Data and the Use of Cookies and IP Addresses
When you visit our site, we do not collect personal data from you, unless you authorize us to
do so. Most of the information we collect is anonymous information such as the pages you visited.
In so doing, we may place a "cookie" on your computer and collect your IP address.

information to properly administer our system, gather aggregate information on visitors to our
site and assess how our site is being used, including the pages visited. We will use your IP
address to personally identify you only when and if we are required to do so by law enforcement
authorities or need to do so to enforce our legal rights.

Member Registration for Personal Claims Data Access
It is our policy to send and receive individualized information through encrypted channels only.
Procedures are established to minimize unauthorized registration of Login ID’s and passwords.
Your Login ID and password may provide access to confidential health information. If anyone other
than you uses your Login ID and password to access PreferredOne’s secured application site, we are
not responsible for such unauthorized access.
